Root canal treatment of a right mandibular first molar with periapical abscess and furcation lesion

Abstract

Root canal treatment (RCT) is the primary therapy for preserving teeth with pulp infections and periapical lesions. This article describes RCT performed on a right lower molar with a periapical abscess and a bifurcation lesion, using the crown-down technique with hand instruments. A 22-year-old woman presented at the UNIMUS Dental Hospital complaining of a blackened and decayed right lower molar, without pain, but with a history of spontaneous pain that subsided after taking analgesics. Radiographs revealed periapical radiolucency and slight radiolucency in the bifurcation area. The diagnosis was established as pulp necrosis with a chronic periapical abscess. RCT preparation was performed using the crown-down technique with large-tapered instruments, irrigation, calcium hydroxide medication, and gutta-percha obturation. Clinical and radiographic evaluation showed no symptoms and evidence of healing of the lesion. It was concluded that non-surgical RCT using the crown-down hand instrumentation technique is effective for cases of periapical abscess involving the bifurcation.
