Periodontal regenerative therapy in diabetes mellitus patient
Terapi periodontal regeneratif pada pasien diabetes melitus
Abstract
Regenerative periodontal surgery has been widely practiced in the management of periodontal tissue destruction cases; the use of biomaterials including bone graft, guided tissue regeneration techniques, matrices and growth factors aims to accelerate the pro-cess of tissue regeneration. The purpose of this case report is to discuss the treatment of a patient with chronic periodontitis ac-companied by DM with regenerative periodontal surgery. A 39-year-old woman came to Unhas Dental Hospital with complaints of right maxillary back teeth since 6 months ago and pain when used for chewing. The patient did not know that she had DM. Fasting blood glucose examination performed showed hyperglycemic condition. Clinical and radiographic examinations show-ed the depth of the pockets of teeth 13, 14, and 15 averaged 8 and 9 mm on the buccal, mesial and distal parts, rocking degree 2, vertical bone damage reaching 1/3 of the tooth root. The patient was then referred to an internist for DM management. Initial treatment began with scaling, root planning and continued with regenerative periodontal surgery at the next visit. Evaluation one week after treatment showed improvement in clinical attachment and reduction in socket depth. The third month evalua-tion, radiographic images showed increased alveolar bone height in the periodontitis-affected region. It was concluded that re-generative periodontal surgery could be a potential treatment in patients with chronic periodontitis with controlled DM.
